Account Setup - Terms of Membership

Group Health Network, Inc. (“GHN”) agrees to confer all benefits of membership in its network unto the individual (“Individual Consumer”) applying hereto.  In consideration for this, Individual Consumer agrees to the following Terms and Conditions of this Agreement.

Healthcare Information:
I, Individual Consumer, desire to have certain services provided to me by Group Health Network, Inc. (“GHN”).  These services will include, but not be limited to, membership enrollment assistance and on-line access to the GHN website in order to assist me in locating and selecting possible healthcare providers.  I understand that there are no Annual Membership Fees, no Reward Redemption Fees, and no fees of any sort charged by GHN.  I understand that some of the information furnished or transmitted to GHN may be related to the provision of medical services and I hereby waive any limitations imposed by the applicable Health Insurance Portability and Accountability Act of 1996 (HIPAA) regulations.  I confirm that I have legal authority to also make this same waiver on behalf of all minors who are joining under my name as my dependents (see "Dependents" section below). I further confirm that I have been given express and knowledgeable consent to make this same waiver, on their behalf, by all adults who are joining under my name as my dependents (see "Dependents" section below).  GHN provides information that has been gathered from sources they believe to be reliable however I am aware that flaws may exist.  I acknowledge that GHN is only an information source and furnisher of Consumer Loyalty Incentive Program (“CLIP™”) services, and that GHN is not a discount card service or any form of insurance coverage.  I further acknowledge that GHN does not provide medical advice and that all medical and health related decisions will be made solely by me upon my consultation with my doctor(s) or other healthcare provider(s).  I am fully responsible for any actions taken based on information from or through GHN.

Membership Eligibility:
I understand and acknowledge that I am applying for membership in the GHN benefits network as an Individual Consumer Member (“Individual Consumer”).  I further understand and acknowledge that my membership eligibility is contingent upon my continued agreement with the Terms and Conditions stated herein.

Dependents:
I, Individual Consumer, understand that each Individual Consumer who meets the minimum eligibility requirements described herein may choose to include an unlimited number of dependents under their own name, at the commencement time of each Individual Consumer’s initial membership.  I further understand that the definition of a dependent includes the following: any and all of Individual Consumer’s self identified family members, regardless of marital status, age, financial relationship, or habitation status.  While group membership benefits are equally available to Individual Consumers and each of their designated dependents, each person individually earns and redeems their own rewards, exclusive of any other person.  There is no sharing allowed in the earning or redemption of rewards.

Reward Points: GHN members will earn Provider Points™ during every visit to their healthcare provider, equaling 10% of total payments (10 points for every dollar) received by their healthcare provider.  Provider Points™ are calculated the same way regardless of how the healthcare provider is ultimately paid and regardless of who actually pays the healthcare provider (e.g., employer-sponsored health insurance plan, individual-sponsored health insurance plan, co-pays, deductibles, self-pay-no-insurance, etc.).  Provider Points™ can be redeemed through a password protected online account for Provider Payment Perks™ certificates, issued in $20 increments.  These Provider Payment Perks™ certificates (originals, not copies) are non-transferable and can immediately be used to offset out-of-pocket costs during the next visit to the same healthcare provider’s office where the Provider Points™ were originally earned.  Liability for sales taxes where applicable and other costs are the sole responsibility of Individual Consumer.  There are no caps or limits on how many Provider Points™ can be earned and Provider Points™ can carry over from year to year.  Rewards cannot be redeemed for cash.  Provider Payment Perks™ certificates are discount coupons that have no cash value.  Please allow 6 to 8 weeks after the last day of each month for Provider Points™ to post to your account.

Group Benefits Offered to All GHN Members: In addition to limitless rewards, deep discounts are also reserved exclusively for GHN members.  Healthcare providers who participate within the GHN benefits network offer special reduced pricing to all GHN members in the form of a discount of 100% off all charges incurred during the initial visit (for new patients).  Healthcare providers who participate within the GHN benefits network will honor all membership benefits described herein and apply them equally to all GHN members (Individual Consumers and their designated dependents), regardless of whether they are a new patient or an existing patient.

In-Network Vs. Out-Of-Network Healthcare Providers:  The Terms and Conditions of this Agreement, including all discounts, rewards, Provider Points™, Provider Payment Perks™ certificates, and other group membership benefits mentioned herein are only applicable to visits made by GHN members (Individual Consumers and their designated dependents) to healthcare providers who participate within  the GHN benefits network.  Visits to healthcare providers who are outside the GHN benefits network do not qualify.

Provision of Service: Each healthcare provider who participates within the GHN benefits network warrants and represents that they are a licensed healthcare professional and that they carry all credentials and insurances required by law within the jurisdictions they practice such services.  I, Individual Consumer, understand that GHN is not a licensed healthcare professional and merely offers consumer loyalty incentive program services.  Accordingly, GHN shall not be responsible for or be party to any litigation arising from medical malpractice or other similar causes of action between healthcare providers and their patients.  I expressly agree to defend, indemnify and hold harmless GHN, its owners, employees, subcontractors, heirs, successors and assigns from and against any and all claims for loss, damages, penalties, fines, liabilities or costs resulting from, associated with, or arising in any manner out of any such healthcare services.

Provider Points™ Earnings Duration: GHN reserves the right to change, rescind, revoke or terminate this Program at any time, with or without cause, and with or without prior notice.  This includes, without limitation, if the terms of the Program do not accurately describe our intent due to errors or omissions.  Unless used or terminated under these Terms and Conditions, Provider Points™ are valid for seven years from the end of the month in which they were earned.  In the eighth year, Provider Points™ earnings accumulated from the first year will expire, and Provider Points™ earnings accumulated in the eighth year will be added in their place, and so on.  Expiration after seven years will occur month-by-month, so seven full years of Provider Points™ earnings are maintained.  Your oldest eligible Earnings are redeemed first.  If GHN terminates this Program, you will have 365 days from the date the Program termination is announced to redeem accumulated Provider Points™ earnings.  If you close your account for any reason, all of your Provider Points™ earnings will expire immediately and will not be available for redemption.  If Group Health Network closes your account for any reason, all of your accumulated Provider Points™ earnings must be redeemed within 90 days thereafter ("Redemption Grace Period").  Provider Points™ earnings not redeemed within the Redemption Grace Period will be forfeited.

Confirmation: I, Individual Consumer, agree that I have read, understand and accept the above Terms and Conditions, as well as all of the Frequently Asked Questions (on the FAQ page) for membership and participation in this Program and this Network.  Further, I understand that If I do not agree to these Terms and Conditions, I will not be granted membership and will not be able to participate in this Program and this Network.  By clicking the "I Agree" tab below, I confirm my agreement.
